I. OBJECTIVE & PURPOSE

To describe the requirement for health insurance coverage for ³ÉÈË´óƬ (UNF) international students with a visa status of F or J, and for students in a program of study with a requirement for proof of health insurance coverage.

II. STATEMENT OF POLICY

1. International Students
   

   All international students with a visa status of F or J  must provide health insurance documentation prior to registration. They may purchase the ³ÉÈË´óƬsponsored group policy or opt to purchase an alternate  health insurance policy. No international student in F or J non-immigrant status shall be permitted to register or continue at ³ÉÈË´óƬwithout providing documentation that the student  has medical insurance coverage for illness or accidental injury that is equal to or exceeds the level of coverage offered by the ³ÉÈË´óƬgroup insurance plan.

2. Non-International Students
   Non-international students in a program of study with a requirement for health insurance must provide health insurance documentation prior to registration. They may purchase the ³ÉÈË´óƬsponsored group policy or opt to purchase an alternate health insurance policy. Non-international students who decide to use an alternate health insurance policy must have active coverage that meets the requirements listed in #3 under III. Statement of Procedures.

III. STATEMENT OF PROCEDURES

1. The minimum requirements for international students in F or J visa status are outlined by Florida Board of Governors Regulation 6.009, Admission of International Students to State University System (SUS) Institutions, at at .
2. International students in F or J visa status who decide to use an alternate health insurance policy will be audited each semester to ensure that the health insurance plan meets or exceeds the level of coverage offered by the ³ÉÈË´óƬgroup insurance plan.
   
   1. Comprehensive benefits covering medical, primary care, prescription, mental/behavioral health, and emergency services
   2. Provider access locally near campus for ongoing care in Florida
   3. A U.S.-filed health insurance plan, not limited to emergency-only, and no out-of-state restrictions on coverage
   4. Must have an unlimited maximum benefit
   5. Without any preexisting conditions limitations
3. Programs of study that require proof of health insurance are reviewed and updated annually.
   These programs are listed on the ³ÉÈË´óƬStudent Health Services website (/brooks/shs/health-insurance.html). The requirement for proof of health insurance is also annotated in the student handbook for the program. Non-international students in a program of study with a requirement for health insurance who decide to use an alternate policy will be audited annually to ensure that the coverage is active and meets the requirements below.
